The Developmental Disabilities Administration (DDA) is pleased to share the Community Supports Waiver Amendment federal application for public review. The waiver program is designed to provide services and supports individuals with developmental disabilities that live in their own home or the home of a family member.
The application is composed of 10 appendixes as noted below. Click on each link to view the proposed track changes for the amendment for each specific appendix.

specifies the administrative and operational structure of this waiver.

specifies the home and community-based waiver services that are furnished through the waiver, including applicable limitations on such services.

specifies the procedures and methods that the State uses to develop, implement and monitor the participant-centered service plan.

describes the methods by which the State makes payments for waiver services, ensures the integrity of these payments, and complies with applicable federal requirements concerning payments and federal financial participation.

Public comments regarding the DDA Waiver Amendments was conducted from February 2, 2019 through March 3, 2019. Comments about the amendments were submitted to DDA by email to email@example.com or by mail to DDA Federal Programs at 201 West Preston Street, 4th Floor, Baltimore, MD 21201.
To support the stakeholder input process and minimize public burden, comments were considered for all of the DDA waivers.
